Musculoskeletal (MSK) ultrasound is a real-time scan that lets us see your tendons, ligaments, muscles, and joints while you move them. Unlike MRI, results are instant — you walk out of the same appointment knowing what's wrong. What MSK ultrasound is
MSK ultrasound uses high-frequency sound waves to create a real-time image of soft tissue and joints. The probe is placed against the skin with a small amount of gel, and the image appears on the screen as you move the affected area. There is no radiation, no contrast injection, no enclosed scanner, and no preparation needed.
It is one of the most powerful tools in modern musculoskeletal diagnosis — particularly for tendons, muscles, and superficial joints — and increasingly preferred over MRI for many soft tissue problems because you can see how the tissue behaves in motion, not just in a static snapshot.

What MSK ultrasound is good at diagnosing
MSK ultrasound is particularly effective for:
Rotator cuff injuries and shoulder impingement.
Tendon problems — Achilles, patellar, tennis elbow, golfer's elbow, biceps.
Bursitis in the shoulder, hip, knee, and elbow.
Joint effusions and synovitis.
Muscle tears, strains, and haematomas.
Ligament sprains and tears, particularly in the ankle and knee.
Nerve entrapment such as carpal tunnel syndrome.
Plantar fasciitis and other foot and ankle problems.
Ganglion cysts and soft tissue lumps.

When MRI is better
MRI is the better choice for:
Deep joint structures (such as the hip labrum or knee meniscus).
Bone marrow problems and stress fractures.
Spinal pathology — discs, nerve roots, the spinal cord.
Cartilage assessment in deeper joints.
Tumour assessment.
If you're not sure which scan you need, we will tell you honestly at your appointment. We have full diagnostic imaging on-site, so if MSK ultrasound doesn't give the answer, we can arrange MRI without sending you elsewhere.

The biggest advantage of MSK ultrasound at Northwest Health is that the scan, the diagnosis, and the treatment plan happen in the same appointment, with the same clinician. Here's what that means in practice:
You don't need a separate referral to imaging. The clinician assessing you is the one doing the scan.
You see the image yourself, as it's done — we'll show you the affected tendon or joint on the screen and explain what we're looking at.
The result is immediate. No waiting for a radiologist's report.
Treatment can start the same day. If an injection is indicated, we can do it under ultrasound guidance in the same appointment.

What to expect at your appointment
Pre-scan history — we'll talk through your symptoms, activity, and any previous imaging.
Clinical examination — hands-on assessment of the affected area.
The scan itself — usually 15 to 30 minutes, with the area exposed and gel applied to the skin. Most scans are entirely painless. We'll often move the area or ask you to contract a muscle so we can see how the tissue behaves dynamically.
Findings explained — we show you the images and explain what they mean in plain English.
Treatment plan — including hands-on physiotherapy if indicated, an injection if appropriate, or referral for further imaging or specialist input.
Written report — a copy of the scan findings and treatment plan, ready to share with your GP, insurer, or sports team.

Ultrasound-guided injections
Where the scan shows that a guided injection is the right treatment, we can do it in the same appointment. Ultrasound guidance ensures the injection reaches the exact target — published evidence shows guided injections are significantly more accurate than landmark-guided ones, with better outcomes and fewer repeats.
We use corticosteroid, hyaluronic acid, and platelet-rich plasma (PRP) injections depending on the condition. Each option is discussed with you before going ahead.

You will not be charged twice. If you book a physiotherapy assessment and the clinician decides ultrasound is indicated, the scan is included in your physiotherapy fee — no upgrade required.

Meet your sonographer — Nathan Briganti
MSK ultrasound at Northwest Health is performed by Nathan Briganti (MSc, HCPC, CSP, ACP), a qualified MSK sonographer and Advanced Clinical Practitioner physiotherapist. His career spans professional rugby with Munster, consultancy with the Philadelphia Eagles in the NFL, outpatient rehabilitation at Hadley Hospital in Washington DC, amputee rehabilitation at Walter Reed Military Hospital, and NHS first-contact physiotherapy in England.
That range matters because diagnostic ultrasound is only as good as the person interpreting the image. Pattern recognition — knowing what normal tissue looks like across thousands of different bodies — is what separates a good scan from a missed finding.

Frequently asked questions

Do I need a GP referral?
No. You can book MSK ultrasound directly. If you have an existing diagnosis or imaging report, bring it with you.

Is the scan painful?
No. Ultrasound is entirely painless — just a probe and a little gel on the skin. We may ask you to move the affected area or contract a muscle during the scan, which can occasionally cause mild discomfort if the area is already painful, but the scan itself is not.

How long does it take?
Most scans take 15 to 30 minutes. Combined with the clinical assessment and treatment discussion, allow 45 minutes for the full appointment.

Is it safe?
Yes. Ultrasound uses sound waves with no radiation. It is the same technology used safely in pregnancy scans and can be repeated as often as needed without risk.

What should I wear?
Loose, comfortable clothing that gives easy access to the area being scanned. For shoulder or elbow scans, a vest or sleeveless top works well. For knee or ankle scans, shorts are ideal. We have gowns available if you'd prefer.

Will my insurance cover it?
Many private health insurers cover MSK ultrasound, particularly when ordered as part of a clinical assessment. We can provide an itemised invoice in your insurer's preferred format. Check your policy first.

What if the scan shows something serious?
If we find something that needs urgent attention or further investigation, we explain it to you immediately and arrange onward imaging or specialist referral on-site, at no additional charge.

How does this compare to NHS MSK ultrasound?
NHS MSK ultrasound is usually only available after a GP referral and an outpatient wait, often several weeks. The scan is performed in a separate appointment from your assessment, and the result is sent back to your GP for action. Our service offers same-week appointments, the scan in the same room as your assessment, and an immediate treatment plan.

Can ultrasound replace MRI?
For many soft tissue and tendon problems, yes — and often gives more useful information because it's dynamic, not static. For deep joints, spine, and bone, MRI remains the better choice. We will recommend the right scan for your specific problem.
