Why Advanced Clinical Practitioner matters
An Advanced Clinical Practitioner (ACP) is a physiotherapist with additional training that allows them to do things a standard physio cannot. Specifically, an ACP can prescribe medication, request imaging directly (such as MRI or ultrasound), perform diagnostic injections, and refer onward to consultants without needing a GP as a middle step.
For you, this means fewer appointments, faster diagnosis, and a single clinician who can manage the whole pathway rather than referring you between different services.
MSK ultrasound — diagnosis at the appointment
Musculoskeletal ultrasound is a real-time scan that we can perform during your appointment. It lets us see tendons, ligaments, joints, and soft tissue while you move, which makes it especially useful for diagnosing rotator cuff tears, tendon problems, joint effusions, and impingement.
We are qualified MSK sonographers, so you don't need a separate referral to an imaging department. The scan is done in the same room as your assessment, and the findings inform your treatment plan immediately.
Ultrasound-guided injections
When an injection is the right treatment — for shoulder bursitis, tennis elbow, knee osteoarthritis, plantar fasciitis, or similar conditions — we use ultrasound guidance to ensure the medication reaches the exact target. Studies consistently show ultrasound-guided injections are significantly more accurate than landmark-guided ones, with better outcomes and fewer repeat procedures.
We use corticosteroid, hyaluronic acid, and platelet-rich plasma (PRP) injections depending on the condition. Each is discussed with you before going ahead, including expected benefits and any limitations.

Nathan Briganti (MSc, HCPC, CSP, ACP) is the founder of Briganti Physiotherapy and a co-founder of Northwest Health. His career spans professional rugby with Munster, consultancy with the Philadelphia Eagles in the NFL, director of outpatient rehabilitation at Hadley Hospital in Washington DC, amputee rehabilitation at Walter Reed Military Hospital, and NHS first-contact physiotherapy in England. He is an independent prescriber and a qualified MSK sonographer.

Frequently asked questions
Do I need a GP referral?
No. You can book directly without seeing a GP first. If you have an existing diagnosis or scan, bring the report with you.
Will my health insurance cover it?
Many private health insurers cover physiotherapy. Check your policy and ask us if you'd like a receipted invoice in your insurer's preferred format. We can also provide treatment summaries for insurance claims.
How many sessions will I need?
It depends on the condition. Simple sprains and strains often resolve in two to four sessions. More complex problems may need six to twelve. We'll give you an honest estimate at your first appointment and review it at each visit.
Will it hurt?
Some manual treatment can be uncomfortable, but never beyond what you're comfortable with. We always work within your tolerance and explain what we're doing and why.
Can I have an injection on the same day as my first assessment?
In most cases, yes — particularly for conditions where the diagnosis is clear and the injection is the right next step. We'll discuss it with you before going ahead.
What if I need an MRI?
If imaging is needed, we can arrange it on-site through our diagnostics service, typically with consultant radiologist reporting within 48 hours.
What's the difference between NHS physio and private physio here?
NHS physiotherapy is usually limited to six sessions of 20 to 30 minutes, often weeks apart, with limited access to imaging or injections without further GP referral. Our service offers longer initial appointments, immediate access to scans and injections, and continuity with the same clinician throughout.
